Why the project matters

In the Mediterranean, non-communicable diseases such as cancer, diabetes, and cardiovascular diseases cause the majority of deaths and healthcare expenses. Hospital-centered models are costly and complex – especially in times of crisis like COVID-19. Prevention and early intervention are critically underfunded, and digital health systems for NCDs are often missing or fragmented. CAREMED addresses this pressing gap by transforming NCD management into a proactive, community-based system, ensuring long-term health improvements and resilience.

• 30 pilot implementations of proactive care models in PHCs across 5 countries.
• Digital Health Information Management Systems piloted and operationalized in 10 PHCs.
• 96 training sessions delivered to healthcare personnel and NCD patients.
• 408 individual care plans and 102 chronic disease registers developed.
• A transnational medical knowledge database and tailored training toolkit.
• Equipment and rehabilitation upgrades in selected healthcare facilities.

• 260+ healthcare professionals trained in innovative care models.
• 40 pharmacists and 28 PHC managers trained in medication management and care coordination.

• Ministries, public institutions, and peer healthcare entities across the Mediterranean through shared policy and training outputs.
