Non-communicable diseases (NCDs) account for 80% of healthcare use and spending in the Mediterranean, affecting already fragile systems. CAREMED responds to this crucial regional challenge by shifting chronic care from hospitals to communities, freeing hospital capacity for more secondary/tertiary healthcare activities. By piloting proactive care models and digital health tools in 30 primary health centers across Italy, Lebanon, Tunisia, Türkiye, and Spain, the project will enhance patients’ health outcomes, reduce hospitalizations and costs, and improve chronic disease management. By implementing an Active Patient Care Model and the Chronic Care Model in 30 Primary Health Centres for over 600 patients in Tunisia, Lebanon, Türkiye, Italy and Spain, CAREMED emphasizes prevention, health education and patient engagement, resulting in reduced hospitalizations and prevented complications.
